Jack Keller, MS, PT
Jack Keller, MS, PT is originally from Rochester, NY, and a graduate of Ithaca College in Ithaca, NY. Growing up in Rochester, NY, Jack enjoyed and played numerous sports including football, basketball and baseball throughout high school. His fascination with strength training and conditioning exercise as a tool to improve athletic performance led him to choose a career in physical therapy. During the course of his studies and training, he developed a passion for manual-based physical therapy techniques as tool to enhance performance and alleviate pain. Following his completion of Graduate School in 1997, Jack chose to leave the snowy winters behind and pursue physical therapy positions in North Carolina.
His career began serving as a statewide traveling physical therapist over the course of 2 years. During this time, he had the opportunity to work in over 30 different clinics throughout the state. It was through this process that he realized that there was a widespread lack of physical therapy treatment available for the person with recurrent or complex musculoskeletal injuries. In 1999, he became self-employed with his focus on providing high quality physical therapy care with an emphasis on manual therapy techniques.
Over time, he combined his physical therapy efforts with other physical therapists and other disciplines of treatment including medical acupuncture, chiropractic care, and massage therapy. From this integrated model, Capital City Health Care Providers, Inc. was formed in 2002. He continues to enjoy the challenges presented by his clients every day.
Away from the clinic, Jack enjoys playing with his two beautiful daughters, exercising, playing golf, and, of course, time with his wife Courtney.
Courtney Keller, MS, PT, PPI
Courtney Keller, MS, PT, PPI, is originally from Lake Hauto, PA, and a graduate of Ithaca College in Ithaca, NY. Following her completion of Graduate School in 1997, she moved to NC to begin her physical therapy career at Nash Health Care Systems in Rocky Mount, NC. In 2002, she joined her husband Jack Keller and helped to begin Capital City Health Care Providers, Inc.
Growing up, Courtney studied ballet for over 10 years and was a member of the Women’s Cross Country and Track and Field teams at Ithaca College. Through these experiences, she gained a keen insight and appreciation of strength training, rehabilitation, and the role of physical therapy. After graduation from Ithaca, she pursued a certification in Pilates Based Rehabilitation to further her skills. She obtained her Professional Pilates Instructor certification in 2003 from the Professional Health and Fitness Institute in Bethesda, MD.
Courtney believes that combined with manual techniques, exercise plays an integral role in healing and injury prevention. In addition to hands on therapies, including joint mobilization and soft tissue massage, Courtney educates the patient to movement therapies including Pilates-based spinal stabilization exercises. She is also the primary therapist responsible for our clinic’s Vestibular Rehabilitation Program.
Courtney enjoys spending time with her family, including her husband and 2 daughters. When not running after her children, she still enjoys going for runs locally or challenging herself on the Wii. She enjoys traveling to the NC Coast with her family for some sun and fun.

As a physical therapist, Kerri believes that manual therapy techniques such as deep tissue massage, joint mobilization techniques, and muscle energy techniques provide the foundation to restore proper joint and soft tissue mobility and reduce pain. Through chiropractic adjustments and nutritional counseling, he was able to stay competitive and was a nationally ranked hurdler.
Dr. Niemchak believes that nutrition plays a fundamental and important role in the reduction and elimination of pain. The ability to provide the body important “whole” food nutrition provides the body the micronutrients necessary for health and wellness.
